Policies
Change of Address:
- Please update the front desk if you have any changes to your address or phone numbers.
Appointments:
- Please notify the receptionist at time of booking if your visit is WCB or ICBC related. Please present your most urgent concern first. The Doctor may require you to book a follow up appointment for other concerns if time does not permit.
- Please call immediately if you cannot make your scheduled appointment time so that other patient's can be booked in that time slot.
NO-SHOW Appointment:
- We require that you to notify us if you are unable to attend your appointment so we may offer that to another patient in need. If you fail to attend your scheduled appointment there is a $35 no show fee.
Behaviour:
- We have a zero tolerance for abuse of any kind towards our staff, other patients and physicians. Disrespectful behavior will lead to dismissal from the practice.
Results:
- We do not provide results over the phone; please book an appointment with your physician to review (patients can also register for “My Health Portal“ at the lab or hospital).
Prescription Refills:
- You must make an appointment with your physician for medication refills. Please plan ahead as our walk in appointments should be reserved for unforeseen medical issues and not routine medication renewals.
Uninsured Services:
- Family Practice Patients Drivers Medical - $100
- Walk In Patient Drivers Medical - $175
- Disability Tax Credit - $150 - $200
- Insurance Forms - vary in price $35 - $150 (Please read the form carefully to see if patient is responsible for costs)
We accept Cash and Debit only.
